Finerenone in Kerendia tablet blocks aldosterone, a hormone that regulates salt and water balance. This leads to lower blood pressure and relieves strain on the kidneys. It can slow down kidney damage in people with diabetes and chronic kidney disease.

Indication of Fineron 10mg Tablet

• Reduces the risk of sustained eGFR decline.
• Prevents end-stage kidney disease.
• Reduces cardiovascular death, nonfatal myocardial infarction, and hospitalization for heart failure in adults with chronic kidney disease (CKD) associated with type 2 diabetes (T2D).

Pharmacology

• Finerenone is a selective, non-steroidal mineralocorticoid receptor (MR) antagonist.
• It blocks MR-mediated sodium reabsorption and MR overactivation in tissues such as the kidneys, heart, and blood vessels, which helps mitigate fibrosis and inflammation.
• Finerenone does not interact with androgen, progesterone, estrogen, or glucocorticoid receptors.
• It lowers systolic blood pressure by about 3 mmHg and diastolic blood pressure by 1-2 mmHg.
• Absorption is complete with an absolute bioavailability of 44%. Peak concentration (Cmax) is reached between 0.5 and 1.25 hours post-dose.
• Steady-state is achieved in 2 days, with steady-state geometric mean Cmax of 160 µg/L and AUCt of 686 µg.h/L at a 20 mg dose.
• Metabolized primarily by CYP3A4 (90%) and CYP2C8 (10%), with 80% of the dose excreted in urine.

Dosage & Administration of Fineron 10mg Tablet

• Starting Dose:
  • eGFR ≥60 mL/min/1.73 m²: 20 mg once daily
  • eGFR ≥25 to <60 mL/min/1.73 m²: 10 mg once daily
  • eGFR <25 mL/min/1.73 m²: Not recommended
• Dosage Adjustment:
  • Increase to 20 mg after 4 weeks based on eGFR and serum potassium levels.
  • Tablets can be crushed and mixed with water or soft food if needed.
  • Monitor serum potassium 4 weeks after initiation and dose adjustments.
• Pediatric: Safety and efficacy not established in patients under 18.
• Geriatric: No dose adjustment is needed for elderly patients.
• Hepatic Impairment: Avoid in severe hepatic impairment (Child-Pugh C); consider additional monitoring for moderate impairment (Child-Pugh B).

Interaction

• Strong CYP3A4 Inhibitors: Avoid use due to increased risk of adverse reactions.

• Strong and Moderate CYP3A4 Inducers: Avoid use as they reduce Fineron efficacy.

Contraindications
